Senior Care Management, Inc. began providing services in San
Diego in 1996. Our staff is comprised of master level social
workers experienced in gerontology,
rehabilitation, home health, and hospice environments. We also
have in depth training and experience in dementia.
Expert consultants in rehabilitation medicine, pharmacy, law,
occupational therapy, physical therapy, speech language pathology,
and fiduciary management complete the care management team.
All care managers
are members of the National Association of Professional
Geriatric Care Managers and the Western Regional Chapter.

About Care Management Professionals
Hilde M.
Lehmann, CMC, President, holds Masters degrees in Social
Work and Public Health from the University of Michigan, with
specialties in Community Organization and Administration,
respectively. Her Bachelor of Arts is from Middlebury College in
Middlebury, Vermont. Hilde has been a vibrant contributor to
health care in San Diego for more than 30 years.
Betsy Evatt,
LCSW, CMC, Director, received her Master of Social Work from the
University of Washington in Seattle with a concentration in Aging.
Her Bachelor of Art is from the University of Notre Dame, South
Bend, Indiana. Betsy has specialized in Social Work in acute rehab
and skilled nursing settings. She also speaks Spanish fluently.
Betsy has served as the co-chair for the San Diego unit of
Professional Geriatric Care Managers (SDGCM). She has written two
published manuals to help organize an elder's personal
information. These products are available on this
website.
April Hayter de Lopez, LCSW, CMC, Care
Manager,
received her Masters and Bachelors degrees in Social Work from San Diego State University.
She has focused her career on serving the needs of the senior population. Her career path has
led her to work in multiple settings including the Office of the Public Conservator, Adult Protective
Services, and the San Diego Mental Health Services' Senior Team.
April currently serves as the co-chair for SDGCM.
Mary Lee
Moser, MA, Creativity Specialist, has a Master’s
Degree in Psychology, and a Certificate in Art Therapy from New
England Art Therapy Institute. She is a certified Geriatric
Activity Leader, and by The Center for Journal Therapy as an
Instructor for Journal to the Self®. Mary Lee has worked
in the health and wellness field since 1986, and has taught
expressive arts in hospitals, hospices, and colleges. She is the
author of There and Back: A Journal Companion for Special
Needs Parents, and a contributor to Writing Routes: A
Resource Handbook of Therapeutic Writing.
