Assessment & Care Plan - Incorporates a
comprehensive, in-home, environmental, psychosocial, safety
assessment with written report and recommendations (if
requested).

On-Going Care Management - This service is ideal
for clients with long distance family or those in need of
additional local support. Regular visits can be provided at
home, in assisted living or in skilled nursing. Emphasis is
placed on communication between client, Care Manager, and
family.

Health Advocate - Care Manager accompanies client
to physician appointments. We take written notes for the
client and family. We provide E-mail or telephonic reports
are provided to designated family members. We schedule all
follow-up appointments, lab work, and assist with
prescriptions. If the client is hospitalized, the Care
Manager will help negotiate the health care system while
keeping the family informed.

Options & Resources – Consultation - The Care
Manager can arrange a one-time, in-home consultation to
identify resources in the San Diego area. Discharge and
placement options can be reviewed. The brief consultation is
available for someone who does not have on-going needs.

Bill
Minder - When a client's vision or other physical
disabilities limits his/her ability to write checks or
manage a checkbook, the Care Manager can help. This service
is typically offered once or twice a month.

"Connections" - This service is geared towards
individuals with no local family available during an
emergency. The client completes an informational
questionnaire and then Care Manager then conducts an initial
assessment. Following this meeting, she maintains contact
with the client by phone twice annually. The follow-up calls
will verify all personal information so the Care Manager is
prepared to assist, when needed.

Safety Assessment - A home safety assessment is
conducted to identify potential problems. Recommendations
emphasize reducing fall hazards due to medical issues and
life style.

End-of-Life Consultation - Assists elders to
understand and complete Advanced Health Care Directives.
Facilitates discussion of final plans. It is often easier
for an objective third-party to initiate this sensitive
discussion.
