is ideal for elders with long distance family or those in need of
additional local support. Includes assessment, development of care
plan, coordination and implementation of agreed upon service
provision.
Family Advocate provides
regular visits to clients at home, in assisted living residences
or nursing homes.
Bill Minder Service
available for assistance with bill paying, checkbook maintenance.
Connections
service is geared towards proactive, independent individuals who realize there is
no family to provide support and assistance if an emergency arises. Care Managers
meet the client and gather sufficient information to step in when (and if!) it is
ever needed.
Home Safety Assessment & Action Plan
utilizes a checklist assessment tool to quickly identify and
implement a home safety plan.
Placement Service
assists clients and families with transition from home to
alternative living arrangements - retirement communities, board
and care homes, assisted living residences, Alzheimer's care
